Prize Draw – Terms and Conditions

York Rehab Associates is celebrating 25 years in business. As a thank you to our past and current patients, from our database of patients, we will randomly draw for 8 selected winners.

This Draw is ONLY open to past and current patients of York Rehab and who are currently residents of the province of Ontario. No entry is required.

No staff of York Rehab or their immediate family can participate in the draw.

Each winner will be awarded a prize of York Rehab’s choosing. There is no cash value for any of the awarded prizes. Winners will be announced via York Rehab’s social media channel.  The maximum number of individual prizes to be awarded is 8.

The prizes are as follows (in no particular order):

Prizes will be assigned to winners randomly. Individuals whose name has been drawn as a winner do not have the option of requesting a prize other than the prize assigned to them. In declining the prize, they forgo being recognized as a selected winner, their name is removed from the draw pool of names and another individual will be selected from York Rehab’s list of patients.

The drawing of names will not be done live.  Selected winners will be contacted via phone or email using the information that York rehab has on file with instructions on how to claim the prize.  If contacted, selected winner must respond within 3 business days of notification or the individual will be disqualified and will not receive the assigned Prize.  If the contact information is no longer valid, no further attempt will be made to locate the individual and the prize will be forfeited.  When selected winners can’t be reached a replacement name will be drawn from York Rehab’s current list of patients.

York Rehab shall have the right at any time to request proof of identity.

All incidental costs and expenses not specifically referred to herein as part of the prize description (the “Expenses”) are the sole responsibility of the prize winner and/or guest(s), where applicable. A winner shall not seek reimbursement for the Expenses from the Sponsor (York Rehab).

Any selected winner contacted who now has a primary address out-of-province will forfeit their winning status and another random name will be chosen from the patient database.

Winners of a prize cannot request that their prize be transferred to another individual, receiving a prize is the sole responsibility of each winner.

To qualify for the draw, the selected winner must be a legal resident of Ontario at the time of being contacted by York Rehab for the purpose of being notified that you have been selected as a winner.

The terms for accepting your prize is that your first name and first initial from last name will be included in York Rehab’s digital marketing channels to notify the public that you have been chosen as a winner.

Winners are to collect their prize by coming into the clinic office at 730 Davis Drive, Newmarket, Ontario. Proof of identity can be requested at any time.  For any winner whose address is within a 20 KM radius of the clinic, the option to deliver may be offered if collecting the prize in person is difficult or inconvenient.  The determination to deliver will be made with the sole discretion of York Rehab.

INDEMNIFICATION BY SELECTED WINNERS.  By accepting a prize, each winner releases and holds York Rehab harmless from any and all liability for any injuries, loss or damage of any kind to the prize recipient or any other person, including personal injury, death, or property damage, resulting in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of any prize, or in any prize-related activity. Each winner agrees to fully indemnify York Rehab from any and all claims by third parties relating to the Contest, without limitation.

Selected winners further agree to be bound by the decisions of the York Rehab, which shall be final and binding in all respects. York Rehab reserve the right, in their sole discretion, to disqualify any selected winner found to be: (a) violating the Contest Rules; (b) tampering or attempting to tamper with the entry process or the operation of the draw, or any related promotional website; (c) violating the terms of service, conditions of use and/or general rules or guidelines of any York Rehab’s property or service; and/or (d) acting in an unsportsmanlike or disruptive manner, or with intent to annoy, abuse, threaten or harass any other person.

York Rehab reserves the right, in their sole discretion, to terminate any Contest, in whole or in part, and/or modify, amend or suspend the Draw, and/or the Draw Rules in any way, at any time, for any reason without prior notice.

The Draw period runs from December 2022 through February of 2023.

These are the official Prize Draw Rules. The draw is subject to applicable provincial and municipal laws and regulations. The Draw Rules are subject to change without notice in order to comply with any applicable provincial and municipal laws or the policy of any other entity having jurisdiction over York Rehab and/or the Draw. All issues and questions concerning the creation, validity, interpretation and enforceability of the Draw Rules or the rights and obligations as between a selected winner and York Rehab in connection with the Draw shall be governed by and construed in accordance with the laws of the province of Ontario.

This Draw is in no way endorsed, sponsored or managed by any social media platforms on which the Draw may have been promoted and/or publicized, including but not limited to Facebook and/or Twitter. Any personal information provided in connection with the Draw is being managed by York Rehab and not sent to Facebook and/or Twitter. Any questions or comments regarding the Draw must be directed to York Rehab and not to any of the social media platforms on which the Contest may have been advertised or administered.
